Pursuant to Law No. 6698 on the Protection of Personal Data, I have been informed about the purpose for which my personal data will be processed by Fuzul G.Y.O, to whom and for what purpose the processed personal data can be transferred, the method and legal reason for collecting personal data and my other rights listed in Article 11 of the Law.

I declare of my own free will that I have read and understood the Communication / Information Form to be processed by Fuzul G.Y.O as the data controller in accordance with the Personal Data Protection Law No. 6698 and other relevant legislation and hereby accept, declare and undertake that I explicitly consent to the transfer of my personal data by Fuzul G.Y.O. to third parties and competent authorities to fulfill the legal obligations of Fuzul G.Y.O.,to have my personal data in the identity and communication category such as name, surname, e-mail, mobile phone provided that it is limited to the purposes and scope, method and legal reasons specified in the "Clarification Text on Personal Data Processed in Communication and Information Request Forms" submitted to persons with whom the Company has signed a contract within the framework of the services received and provided by the Company and third parties with whom they cooperate with, to promote products and services, inform me about campaigns and promotions, send me notifications containing advertisements via various digital communication channels such as e-mail, SMS and telephone and that I have read and understood the clarification text.
